Case
Management |

Case Management is a strategy for helping a family and/or
individual, who faces multiple problems to design and
implement a set of "customized" steps that will lead
ultimately to a solution of his/her problems over time, and
reach self-sufficiency. Case management receives funding
through the
Homeless Assistance Program (HAP) and the
Human Services Development Fund (HSDF).

To provide comprehensive
services, advocacy and guidance to homeless/ low income
