Case Management

 

Case Management is a strategy for helping a family and/or individual, who faces multiple problems to design and implement a set of "customized" steps that will lead ultimately to a solution of his/her problems over time, and reach self-sufficiency. Case management receives funding through the Homeless Assistance Program (HAP) and the Human Services Development Fund (HSDF).

 

Case Management is defined as:

  • Consumer Centered

  • Engaging

  • Assessing

  • Planning

  • Accessing Resources

  • Coordinating

  • Monitoring

  • Disengaging

 

Mission

To provide comprehensive services, advocacy and guidance to homeless/ low income families and individuals in order to help them reach their goals towards self-sufficiency and increase their involvement in the community.
